The NHS has introduced a new service for people who are more at risk of an unplanned hospital admission and who would be likely to benefit from more tailored, active support from their GP surgery.
As a member of this programme, patients will have a named GP who will have overall responsibility for the care and support that our surgery provides. Patients will also have a named Care Co-ordinator.
We hope this will help to provide continuity of care for our patients who are more at risk of an unplanned hospital admission. If we identify you as a patient who may benefit from this new service we will write to you to tell you this and who your named GP and Care Co-ordinator is. These new arrangements do not prevent patients from seeing any other clinician working at Shires Healthcare.
